Strengthening the Response to Diabetes & Hypertension in Herat
Project Overview
We’re improving early detection, referral, and basic management of diabetes and hypertension in Herat by upskilling frontline health workers and activating community screening and counseling.
Objectives
• Train and support health workers and community health workers (CHWs) on screening, counseling, referral, and follow-up.
• Equip facilities with simple tools and practical record-keeping to track NCD services.
• Use routine supervision and data reviews to keep quality improving.
Target Audience
People at risk of diabetes and hypertension in Herat—reached through public clinics and CHWs in surrounding communities.
Key Activities
• Cluster-based trainings for clinic staff and CHWs.
• Community screening and counseling with clear referral to nearby clinics.
• Monthly supervision visits and simple, paper-based service tracking.
• Regular review meetings to identify gaps and take corrective actions.
